What counts as scope creep?
Any work that ends up inside a project without being added to the quote. Extra rounds of revisions, a page that was never specified, a meeting that becomes three. The hours are real even when the fee does not move.
How is the cost worked out?
Your quoted rate is the fee divided by the hours you quoted. Every hour beyond the quote is valued at that rate, because that is the rate you agreed to work for. Multiply those unpaid hours by the quoted rate and you have the cost of the overrun.
How do I stop it happening on the next project?
Decide the hours before the work starts, keep them attached to the project, and watch what is left as the weeks pass. When the remaining hours stop covering the remaining work, that is the moment to talk to the client, not after the overrun.
